- Vendor Liability Insurance: All vendors must submit proof of insurance. If a vendor doesn’t already have a policy, we recommend the following:
- Storage Waiver Form: For full time food & farm vendors only.

- Farmers Market Food Vendor Application: For packaged foods.
- Farmers Market Food Establishment Application: For cooking and preparing food on-site
- Farmers Market MO Farmers Only Registration Form: For Missouri Farmers
- Farmers Market Non-Missouri Farmer Application: For out of state farmers.
- Farmers Market Commissary Agreement: For vendors who prep food before market.
- Alcohol Vendors: Will need state and county Liquor Licenses. MAFM provides the U City Temporary Permits. Contact the market manager for more information
- Egg Vendors: Must submit MO Egg Retailers License. Get license here: https://apps.mda.mo.gov/webforms/Egg-License-Form/
